Telehealth informed consent
Effective date: 01.19.2024
This Telehealth Informed Consent describes the nature of telehealth services provided by licensed providers through the MD•8 platform. By requesting a consultation, you acknowledge that you have read and agree to this consent.
What telehealth is
Telehealth is the delivery of healthcare using electronic communications — including secure questionnaires, messaging, images, audio, and video — where the provider and patient are not in the same physical location. Your MD•8 consultation may be conducted asynchronously, meaning your provider reviews the information you submit and responds without a live visit.
Expected benefits
- Access to a licensed provider without traveling to an office.
- Convenient, same-day review of your health questionnaire in most cases.
- Ongoing messaging with your Care Team for follow-up and dose adjustments.
Possible risks
- Information transmitted may be insufficient or of poor quality, which could affect clinical decision-making.
- A physical examination and in-person laboratory testing are not performed, so some conditions may go undetected.
- Technology failures or delays may interrupt care, and in rare cases electronic security protocols could fail.
Your responsibilities
You agree to provide accurate, complete, and current information about your health history, medications, allergies, and symptoms. You agree to follow the treatment plan and dosing instructions provided, to report side effects promptly, and to seek in-person care when clinically necessary.
No guarantee of treatment or results
Submitting a request does not guarantee that a prescription will be issued. Your provider may determine treatment is not appropriate or may recommend in-person evaluation. Individual results vary, and compounded medications are not approved or evaluated for safety, effectiveness, or quality by the FDA.
Privacy of your information
Your health information is handled as described in our Privacy Policy and, where applicable, in your provider's Notice of Privacy Practices. Reasonable safeguards are used to protect the confidentiality of information transmitted through the platform.
Withdrawing consent
You may withhold or withdraw this consent at any time without affecting your right to future care or treatment. To withdraw consent, message your Care Team or email boaz@md8.fit.
Emergencies
Telehealth is not appropriate for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
